Advanced Healthcare Directive
An Advance Healthcare Directive expresses your preferences on general, critical and end-of-life treatment and care. This legal document goes into effect only if you are incapacitated and unable to speak for yourself. This could be the result of disease or severe injury—no matter how old you are. It helps others know what type of medical care you want.
This ADVANCED HEALTHCARE DIRECTIVE must be in writing, signed and dated by YOU in front of two (2) witnesses, signed by your Proxy and signed by your second Proxy choice. Keep in a safe place.
Remember: If you do have an advance directive, be sure that your doctor, family, and friends know you have one and know where it is located. As long as you can speak for yourself, you can change your mind any time about what you have written down. If you make changes, tear up your old papers and give copies of any new forms or changes to everyone who needs to know.
